A community health organization is seeking a Community Health Worker (CHW) to improve patient health through social support and advocacy. The CHW will perform needs assessments, assist with medical appointments, and provide health education to manage chronic diseases like diabetes and hypertension. Applicants should have a high school diploma or GED, a valid driver’s license, and demonstrate a strong commitment to community health.

Bilingual (Spanish) candidates are preferred. This is a temporary, onsite position in Camden, NJ.
