Responsibilities
  • Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
  • Ensure all nursing personnel adhere to the facility 0policies and procedures; periodically review and revise as needed.
  • Plan shifts 0services, programs, and activities with assigned nursing staff and support personnel.
  • Maintain current Nursing Service Procedures Manual and ensure staff compliance with its procedures.
  • Provide written and oral reports/recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain the resident 0total regimen of care.
  • Participate in the facility 0quality assurance program and facility inspections as requested.
  • Assist in planning the nursing services portion of discharge plans as necessary; admit, transfer, and discharge residents as required.
  • Complete accident/incident reports as necessary; manage charting and documentation per policies.

Charting and Documentation

  • Complete and file required records at admission, transfer, discharge, and ongoing care, ensuring compliance with the medical records system (electronic or traditional).
  • Encourage physicians to review and sign orders and progress notes; transcribe orders to resident charts and care plans.
  • Document nursing notes accurately, file accident/incident reports, and maintain the Daily Census Report.
  • Record diet changes and communicate with Dietary Department; report discrepancies to the Nurse Supervisor.
  • Ensure documentation of changes and adherence to physician orders, with timely sign-off by the appropriate staff.

Drug Administration Functions

  • Prepare and administer medications as ordered; verify resident identity before administration.
  • Maintain adequate stock of floor medications, supplies, and equipment; report needs to Nurse Supervisor.
  • Order medications, supplies, and equipment per policy; maintain narcotic records and address discrepancies with Nurse Supervisor.
  • Review medication charts for accuracy and adherence to policies; notify physicians of stop orders when applicable.

Qualifications

  • Education: Must be a graduate of an approved LPN/LVN program; continuing education desirable.
  • Experience: On-the-job training provided.
  • Specific Requirements: Current, unencumbered LPN/LVN license in the state; proficiency in English; ability to make independent decisions; strong interpersonal skills; knowledge of long-term care laws, regulations, and guidelines; ability to plan, organize, and implement programs; patience and enthusiasm; willingness to adopt new practices.
  • Physical and Sensory Requirements: Move, lift at least 25 pounds; stand and walk; ability to cope with mental and emotional stress; vision and hearing adequate; capable of working with ill and elderly residents; capable of using office and clinical equipment; emergency evacuation capabilities as needed.

Safety, Equipment, and Additional Functions

  • Adhere to safety and sanitation policies; participate in infection control programs; use PPE as required.
  • Ensure proper use and disposal of equipment; participate in fire safety and disaster drills; assist with hazardous condition reporting.
  • Provide data to Quality Assurance & Assessment Committee as requested.

Working Conditions

  • Work in office areas and throughout the nursing service areas; frequent interruptions and exposure to residents, families, and staff; potential for exposure to infectious materials and hazardous chemicals.
  • Requires shift work, occasional overtime, and participation in continuing education; may be involved in evacuations during emergencies.

Miscellaneous

  • Must not pose a direct threat to others; must maintain confidentiality of resident information; comply with advance directives policies as applicable.
