Social Care Practitioner – Cornwall Council Job Description
Under the leadership and management of the Team Leader, you will work within a team of social workers and other social care practitioners to provide a service to adults with health and social care needs. You will achieve high quality, timely and person‑centred needs assessments, plan and work to improve outcomes, and increase or maintain the independence of vulnerable adults.
Responsibilities- Work collaboratively with colleagues from different teams, disciplines and partner organisations to provide seamless services.
- Protect vulnerable adults from harm or abuse and support people to optimise their choices for independent living.
- Promote personal strengths of individuals, families and communities.
- Handle public/customer facing duties, subject to statutory English language requirements.
- Assist in roles subject to an enhanced criminal record disclosure check.
- Minimum Level 3 qualification in health, social care or a relevant area, or willingness to work towards appropriate qualifications.
- Effective interviewing skills to undertake assessments of need, including risks and strengths.
- Demonstrable skills in analysing information and translating it into a coherent plan and outcome.
- Creative, resilient under stress, ability to ask for support and to refer cases to more experienced or qualified workers as required.
- Ability to reprioritise and remain flexible to respond to urgent referrals.
