Overview
Case Manager for the Moorpark Student Health Center (SHC), reporting to the SHC Coordinator and under the supervisory psychologist. This is a temporary, grant‑funded position supported by the California Community College Chancellor’s Office Mental Health grant.
Work Responsibilities- Provide case management and referral services to a diverse student population, including individual meetings in person, by phone, video, or email.
- Coordinate and schedule post‑hospitalization appointments and ensure students are connected to appropriate on‑ or off‑campus treatment providers.
- Assess current needs of students with prior psychiatric hospitalization, develop treatment plans, and collaborate with SHC clinicians when necessary.
- Research and maintain connections with community mental health resources, sliding‑fee‑scale agencies, and other health services, and update mental health pathways for Ventura and North Los Angeles County.
- Assist students in navigating the health‑care system for mental and physical health resources.
- Track and follow‑up with students for 2–4 weeks after providing resources to ensure continuity of care and prevent students from falling through the cracks.
- Obtain release of information and necessary records from SHC medical/mental health teams after psychiatric hospitalizations.
- Help students with complex mental health needs connect with off‑campus providers of service.
- Provide limited direct crisis management services to students in distress.
- Research national mental‑health trends, implement safety protocols, and support campus‑wide mental‑health programs.
- Coordinate with student service programs and campus collaterals to ensure supportive programs for student success.
- Collaborate with the Basic Needs Center to assess and assist foster youth and homeless students with mental‑health care, housing, and financial resources.
- Organize trainings and informational sessions with community mental‑health providers (e.g., Coalition for Family Harmony, RISE program, TAY).
- Maintain weekly scheduled appointments and documentation in electronic records systems, ensuring accuracy and timeliness.
- Communicate clearly, respectfully, and professionally with students, staff, and faculty.
- Demonstrate sensitivity and cultural competence when working with students of diverse racial/ethnic backgrounds, sexual orientations, and disabilities.
- Other duties as needed or requested by the SHC or College administration.
- Master’s degree in Clinical Psychology or Social Work (preferred).
- Bachelor’s degree in Clinical Psychology or Social Work.
- Minimum 2 years of experience in a similar case‑management or mental‑health position.
- Demonstrated experience in crisis and behavior risk assessment.
- Experience with crisis intervention and management.
- Knowledge of community agencies and mental‑health resources.
- Experience planning mental‑health outreach and prevention activities.
Professional Expert Contract, non‑benefitted, grant‑funded position.
Master’s holders: $60/hour, 20 hours/week, 12 months.
Bachelor’s holders: $45/hour, 20 hours/week, 12 months.
