We are seeking a dedicated Licensed Vocational Nurse (LVN) for a Skilled Nursing Facility (SNF) assignment. The LVN will work under the direction of the Director of Nursing and Charge RN to provide high-quality, individualized patient care while collaborating with physicians, nursing staff, residents, families, and interdisciplinary teams.

Scrub Requirement: Caribbean Blue scrubs are required for this assignment.

• Perform comprehensive patient assessments and report abnormal findings
• Administer medications and treatments per physician orders and facility policy
• Document patient conditions, responses to care, and progress accurately
• Assist with emergency response and resuscitation efforts as needed
• Provide patient and family education
• Maintain safety, infection control, and confidentiality standards
• Collaborate with nursing staff and assist other departments as needed
• Participate in shift handoff rounds and continuity of care
• Support orientation and education of new staff when applicable

• Active California LVN License
• BLS (CPR) Certification required
• Minimum 1 year of SNF or relevant nursing experience
• Ability to lift, bend, walk, and perform required nursing duties
• Strong assessment, communication, and teamwork skills
• Dependable with a strong attendance history
