Licensed Vocational Nurse (LVN) – Skilled Nursing Facility | Contract

We are seeking an experienced Licensed Vocational Nurse (LVN) for a 13-week contract assignment in a Skilled Nursing Facility (SNF). This role is ideal for LVNs with strong long-term care experience who are confident in medication administration, resident care coordination, and team supervision.

Key Responsibilities:

• Assess, plan, and implement nursing care for residents under the supervision of a Registered Nurse
• Administer medications and provide direct patient care in accordance with care plans
• Ensure continuity of care through accurate documentation and communication
• Supervise and support CNAs and other support staff
• Monitor resident conditions and report changes promptly
• Maintain compliance with facility policies, procedures, and regulatory standards
• Utilize electronic medical records accurately and efficiently

Required Qualifications:

• Active California Licensed Vocational Nurse (LVN) license
• Minimum of 2 years of skilled nursing or long-term care experience (required)
• Proficiency with Electronic Medical Records (Point Click Care  experience preferred)
• Current BLS certification
• Ability to pass required nursing competency/medical testing
• Strong verbal and written English communication skills
• Professional, respectful, and collaborative approach to patient care

• Strong clinical judgment and attention to detail
• Effective interpersonal and leadership skills
• Basic computer proficiency
• Ability to work independently and as part of a multidisciplinary team
