Requirements:

• Experience: 2 years Acute Care Hospital Case Manager experience in Care Coordination & Discharge Planning (Hospital experience required)

• Certification: BLS

• Charting System: Epic

• Patient Age Group: Geriatrics

• License: CA RN License (Pending accepted but must be active by start)

• Ratios: 1:20

• First-Time Travelers: Yes (clinical interview)

• RTO: Max 7 days
