• Deliver high‑quality, person‑centered care for residents with chronic conditions, post‑acute needs, and complex medication regimens.
• Assess health status, identify risk factors, and lead the development and ongoing refinement of individualized care plans in collaboration with physicians, therapists, social workers, and families.
• Administer medications and treatments with precision, monitor responses, manage wounds and IV therapy as appropriate, and implement fall‑prevention and safety protocols to uphold dignity and independence.
