Maternal Child Health/Family Health Nurse

Sawtooth Mountain Clinic is looking for a caring, supportive, and knowledgeable registered nurse to take over the role of MCH/Family Health Nurse.

Sawtooth Mountain Clinic’s (SMC) Maternal Child Health / Family Health Nurse is an essential member of an integrated team that provides care to prenatal and postpartum mothers, families, and children in our service area. The Maternal Child Health / Family Health Nurse provides care coordination to pregnant families, coordinates the Women, Infants, and Children (WIC) program, coordinates the Family Home Visiting program, and provides other services such as lactation support and childbirth education.

The work of this role makes a difference in families’ lives, supporting families in providing healthy environments for their infants. Strong communication and organizational skills are integral to this role.

Responsibilities are as follows:

• Coordinates Women, Infants, and Children (WIC) Program for Cook County, including reporting, direct participant service provision, and oversight of part-time WIC CPA and WIC Breastfeeding Peer Support staff
• Provides Care Coordination for Pregnant Women as part of SMC’s integrated care team, including providing prenatal education and lactation support as needed
• Coordinates Cook County’s Family Home Visiting Program, including direct service provision as well as reporting
• Maintains strong rapport with SMC Primary Care Providers and other members of SMC’s care team, ensuring clear coordination of patient care via thorough documentation and consultation
• Supports local Childbirth Education efforts as needed
• Participates in interdisciplinary meetings as assigned
• Other Family Health responsibilities as assigned

Qualifications:

• Valid Minnesota RN Nurse License
• PHN Certification preferred
• Program Management experience preferred
• Previous Maternal Child Health experience desirable
• Excellent communication skills, both written and verbal
• Desire and ability to work as an integrated part of a care team

Salary: $33.00-$36.00 per hour, commensurate with educational background and experience
