Job Description

Schedule – Part-time Nights 2 days per week, 7p-7a with rotating weekends.

Responsibilities
  • Identifies and formulates expected outcomes based on diagnoses, ensuring they are realistic, attainable, and culturally appropriate.
  • Develops individualized plans of care in collaboration with the patient, family, and healthcare providers, ensuring continuity and documentation.
  • Implements interventions from the plan of care safely, timely, and appropriately, documenting actions taken.
  • Evaluates patient progress towards outcomes, revising diagnoses, outcomes, and plans of care as needed.
  • Administers medications and treatments as prescribed, ensuring patient safety and adherence to protocols.
Knowledge, Skills, and Abilities
  • Administers all types of medications correctly per age group for all populations served according to policy
  • Demonstrates proficient technical skills and operational knowledge of all equipment used on the unit, including equipment used to stabilize and monitor patients (e.g., IV insertion, central lines, pulse oximetry, NG tubes, Foley, chest tubes, hemodynamics monitoring)
  • Proficient in the use of electronic medical records and automated dispensing medication systems for efficient medication administration and patient data retrieval
  • Performs daily rotational task assignments as directed
  • Supervises Nursing Assistants in basic nursing functions, ensuring adherence to the patient’s cultural needs and safe patient ambulation, transfers, and repositioning
  • Preceptor duties as necessary to guide and instruct less experienced staff
  • Engages in effective communication and collaboration with the healthcare team, including gathering and reassessing patient data, initiating and implementing physician orders, and contributing to care plan development
  • Understanding of emergency codes, infection control protocols, life safety, fire plans, and age-specific safety concerns; supervises nursing assistant staff during these events
  • Provides education to patients and families from admission through discharge, covering medical and nursing plans of care, medications, tests and procedures, disease processes, and discharge instructions. Documents education provided, including patient responses and outcomes
  • Initiates referrals to healthcare providers as necessary
  • Fluent in English with excellent reading, verbal, and written communication skills for effective interaction with patients, families, and the healthcare team
