The Program

As a poverty reduction program, Patch delivers services to people living with low‑income, in an integrated, wrap‑around fashion, at place‑based neighborhood hubs located in Calgary Housing areas. Services reflect the expressed and assessed needs, priorities, and interests of participants – individuals, families, youth, and children. Patch services target the multi‑dimensional nature of poverty for people.

The Position

Patch is looking for relief Community and Family Facilitators (CFF) for their Bannister (South), Shaganappi (West), West Dover (SE), and Woodridge (SW) offices. Patch is open Monday through Thursday from 9-5 and Friday from 9-4.

We seek candidates who can best fulfill the duties and responsibilities identified below. This is an excellent developmental opportunity for someone seeking experience in a dynamic community program.

The Community and Family Facilitator is part of a team responsible for the day‑to‑day operations and walk‑in appointments at the Patch hub and will deliver services for Basic Needs & Referrals as well as support in Employment & Education, Financial Empowerment, or Community Connection.

All positions will be trained in skills for Psychological Recovery (SPR) and Natural Supports Framework.

Duties And Responsibilities
  • Deliver Patch services on‑site, in the community, one on‑one, in group settings, over the phone and virtually.
  • Engage with individuals and families about their situation, build relationships, support with stress reduction, and introduce strategies to increase thriving through other Patch services or referrals to other community supports and services. Provide services to assess need, assist individuals with prioritizing areas of focus, and provide support and interventions accordingly.
  • Promote the well‑being and mental health of participants, this includes providing general crisis support, teaching coping skills that help people develop skills and managing emotional and behavioral reactions.
  • Provide supports and services to participants that aim to increase access to resources and enhance support systems, encouraging the development and use of natural supports.
  • Provide support to participants to meet their basic needs (i.e., food, housing, transportation, employment, daycare, social support networks).
  • Work collaboratively with surrounding community resources.
  • Keep thorough and up‑to‑date records of contact with participants and the services provided.
  • Travel to and from various community resource providers and services.
  • Be prepared to work at any of the Patch hub locations to offer support and provide coverage as required and directed.
