Resource Coordinator (Full Time)

Reporting to the Family Resource Manager, the Resource Coordinator is responsible for providing direct service to the disability community by supporting individuals, families, caregivers, and professionals through fostering resilience, independence, and inclusion within our community, ensuring every family has the resources, funding, equipment, and knowledge they need to navigate obstacles, build strong support networks, reduce vulnerability, and advocate for themselves and their loved ones.

This position will provide direct service support through Easter Seals Alberta Family Resource Center programs of Accessibility, Family Support, PDD and FSCD Training.

Responsibilities
  • Serve as a central point of contact for individuals, caregivers, and professionals within the community.
  • Ensure intakes are reviewed, prioritized, and managed.
  • Guide individuals, caregivers, and professionals through needs-based assessments and co-creation of a service plan to meet the determined needs.
  • Support individuals, caregivers, and professionals with applications pertaining to Income Supports, housing, adaptive equipment, guardianship, trusteeship and more.
  • Provide resources or referrals to alternative adaptive equipment supports and arrange equipment loans, complete and process documentation, and equipment delivery.
  • Maintain accurate and up-to-date records of client documents, service plans, progress notes, and correspondence.
  • Seek appropriate consultations (internal and external) to ensure the best support is provided to individuals, caregivers, and professionals.
  • Provide support to families with navigating available community resources.
  • Ensure follow-up procedures are maintained to ensure individuals, caregivers, and professionals have access to information and resources within a timely manner.
  • Identify resource gaps and communicate with the Family Resource Center Manager.
  • If or when a crises call is received, offer emotional support, practical assistance, and linkage to appropriate services to help stabilize the situation and prevent further escalation. Connect with Family Resource Center Manager for support.
  • Support Family Resource Specialist with Peer Connection Support & virtual programs.
  • General data entry and telephone answering.
  • Attend events as required to support and promote agency.
  • Other duties as assigned.
Qualifications
  • A diploma/degree in Social work or related field is an asset
  • 2 years of experience in needs based assessments and facilitating resource navigation support.
