McMan is recruiting for a Case Manager to join our team providing a housing first and harm reduction approach; the Case Manager will be responsible for assisting residents in maintaining their housing, assisting accessing financial supports, supporting their education/employment goals and any other areas of support that is required to help them successfully transition into independence. The Case Manager will work alongside our Transition Specialists, by being a member of the resident’s team that is planning around the needs for their successful transition into the community.

Key

aspects of this role include:
  • Educate and support individuals to increase skills that promote greater self‑efficacy.
  • Provide individual instruction, modelling, and support of basic life skills (e.g. health, nutrition, home cleanliness, safety).
  • Monitor and support individuals through a harm reduction approach to ensure living conditions are safe and sanitary.
  • Implement integrated service delivery from the point of referral to program completion and transition for individuals accessing the program.
  • Complete case management responsibilities that include assessments, goal planning, case consultation, monitoring and evaluation of the goals to meet the client’s needs.
Required qualifications:
  • A degree or diploma in Human Services.
  • Three (3) years’ experience working with vulnerable populations.
  • Strong writing, organisational, programme planning and interpersonal skills.
  • Understanding of Rapid Rehousing.
  • Demonstrated interest and knowledge of the Homeless Serving System of Care.
  • Experience with HMIS data preferred.
  • Current standard First Aid Certification and Suicide Intervention Training.
  • Valid Driver’s licence and adequate insurance coverage.
  • The successful candidate must also have a Child Welfare Intervention Check and Police Check (current within 6 months).

* This role will require on‑call support on a rotating basis and some evening and weekend work hours.

Salary Range: $49,768.00 – $54,931.00 annually
