McMan is recruiting for a Transition Specialist to join our team working from client centred and strength‑based approach, utilizing the Natural Supports and Trauma Informed Practice frameworks. The Transition specialist supports tenants of the James House program who are exiting homelessness to develop their service planning and natural support teams with the goal of finding safe and stable housing while exiting the homeless serving systems of care.
The Transition Specialist will also provide support to the client once they have moved into independent living in the community for three months to support safety and transition plan implementation.
Some of the key aspects of this role include:
- Identifying, engaging, strengthening, and expanding natural support networks
- Developing integrated plans that address priority needs of the individual
- Developing comprehensive crisis stabilization and relapse prevention plans
- Facilitating and coordinating team members from various social service systems
The successful candidate will have:
- A Degree/Diploma in Human Services or similar
- Demonstrated skill set in working with individuals experiencing homelessness
- Understanding of and ability to work with numerous social service systems
- Awareness of appropriate community resources and services
- Strong writing, organizational, program planning, facilitation, and interpersonal skills
- High Fidelity Wraparound training or experience
- Knowledge of diversity issues around culture, religion, and other orientations
- 3 to 5 years experience facilitating teams and working with various systems
- A current (no more than 6 months old) Police/Vulnerable Sector Check and Child Intervention check.
- Have a reliable vehicle and proof of adequate vehicle insurance
* This role requires flexibility in working hours and support evening work with clients.
* This role will require on call support on a rotating basis and some evening and weekend work hours.

Salary Range:$57,384.00- $63,761.00 annually
