PositionSummary
Reporting to the Manager, Medical Clinic & Patient Access, the Nurse Practitioner is responsible for providing health services to manage the needs of individuals, families, groups, and communities. The NP uses principles of primary health care to guide clinical practice and contributes to the models of health care established by Mosaic PCN.
The NP provides comprehensive assessments, orders and interprets diagnostic and screening tests, diagnoses illness and disease, and therapeutically manages health concerns, including prescribing medications and therapies. The NP works autonomously and collaborates with the health care team to deliver comprehensive services.
This position works in Low-Risk Maternity Group (LMG), Mosaic Medical Clinic (MMC), ACCESS clinic and the Mosaic Pain Management Clinic (PMC). When working at the PMC, NPs collaborate with the multidisciplinary team to provide detailed assessment and comprehensive health services for patients with chronic pain.
Key ResponsibilitiesHealth Assessment and Diagnosis
- Performs an advanced comprehensive and focused health assessment.
- Synthesizes and analyzes health assessment information using critical thinking and clinical reasoning skills.
- Combines patient assessment findings with scientific and clinical knowledge to diagnose within the NP scope of practice.
- Orders appropriate screening and diagnostic investigations and interprets results based on clinical reasoning and evidence.
- Consults with and refers to physicians or other health care team members when patient needs are beyond the NP scope or competence.
Health-Care Management and Therapeutic Intervention
- Collaborates with patients, families, and the health care team to share decision making and set priorities for management of health concerns.
- Applies pharmacology knowledge in selecting, prescribing, monitoring, and dispensing drugs within the NP scope of practice.
- Manages treatment for patients diagnosed by the physician or NP within their scope of practice.
- Initiates interventions to stabilize clients in urgent or emergency situations.
- Educates patients on pain science and biopsychosocial approaches to managing chronic pain in alignment with current pain science and the PMC team messaging.
Health Promotion and Prevention
- Includes health promotion, illness prevention and capacity building in client relationships and responses to health needs.
- Partners with other health care providers to plan and develop health promotion/illness prevention programs based on client needs and evidence-based approaches.
- Advocates for environments that facilitate client learning and participation in meeting their health needs.
Documentation
- Documents all phases of patient care in the EMR and MPCN evaluation systems.
- Participates in quality improvement activities.
Professional Role & Development
- Explains and promotes the NP role to patients, the public, and healthcare professionals.
- Works within regulatory acts, regulations, bylaws, and evolving scope of practice for the NP role.
- Adheres to professional and legal standards and ethical decision-making in diagnosis and treatment, including prescribing medications.
- Identifies and pursues professional development needs.
