The company:

Partnership of Care Supported Living provides community based support services for adults with learning disabilities, autism and behaviours of concern . Services offered are residential care, transitional support day service and supported living. The company is registered with the CIW and complies with all care regulations.

Purpose of Role:

To provide a safe and quality service within the local area. This will involve delivering direct support to individuals who have learning disabilities, autism and behaviour of concern in a way that enables them to maximise their independence, maintain family relationships and access community opportunities. To represent the organisation in a positive way through when interacting with external partners including purchasers, clients, carers, families and communities and to contribute to Area and Organisational objectives.

To be accountable for your practice and development of your skills and knowledge.

Dimensions of the Role:

The role will be based in the Caerphilly county borough area, the role will need to reflect the provision of 24/7 care as necessary and will include working over a 24/7 period. The role also includes being on call for your own project

Main duties & responsibilities:
Major Objectives:
Client Support:
  • To ensure that the rights of the client are upheld and promoted at all times in accordance with the principles of choice, respect, dignity and privacy.
  • To ensure that all issues relating to the safeguarding of vulnerable adults are reported according to policies and procedures and in accordance with the safeguarding policy.
  • To ensure that if there are significant changes to individual clients needs these are raised with the services co‑ordinator.
  • To ensure that all client records are maintained in accordance with policy and procedure.
  • To update the clients files every six moths
  • Too produce monthly reports regarding the individual clients.
  • To analyse BOC charts monthly and then every quarter produce behaviour report.
  • To ensure that all support is delivered in accordance with risk assessments, care plans and behaviour management plans that are in place and to contribute to the regular review of such documents as appropriate.
  • To take the lead and support less experienced staff to work with more complex clients and to manage crisis situations in services.
  • To bring to the attention of the Supported living deputy and/or Supported Living Manager any concerns or deficits in the provision of care.
  • Too arrange health care appointments and also to attend these appointments .
  • To strive to ensure clients satisfaction and to report complaints and concerns in accordance with policy and procedure.
  • To promote client involvement in all areas including service delivery, quality assurance and organisational development.
  • To support the effective management of client finances ensuring that all transactions involving client finances are recorded, verified and audited in accordance with policies and procedures.
  • To support clients with holidays of their choice.
Staff Management:
  • To proactively coach and develop less experienced support staff in providing a high quality service.
  • To ensure that company policies and procedures are understood and followed by all Support Workers.
  • To report any incidents that require investigation under disciplinary and grievance procedures immediately.
  • To undertake staffs supervisions every two months and an banding appraisal every six months, and then an annual appraisal
Organisational and Administrative:
