RESIDENTIAL HOME SUPERVISOR - SENECA PLACE HOME

Assumes primary administrative responsibility and authority for up to six (6) adults with developmental disabilities or mental illness in a specialized residential home, under the supervision of the Residential Services Administrator (RSA). Responsible for maintaining the 24-hour operation of the home and directing the activities of the residential care home staff in providing training, maintaining care and safety, and assuring the rights of persons living in the home.

Responsibilities will also include maintaining relationships between NLCMHA and ancillary support organizations; and advocating for residents of the home.

Essential Functions
  • Assume responsibility with consultation of the RSA, in matters of staff recruitment and hiring. This will include appropriate completion of the application process, review of job qualifications and job descriptions, conducting interviews, checking references, coordinating employment contingencies, coordinating with area physicians for scheduling of employee physical examinations, and obtaining verifications for employment eligibility, driver’s license and licensing record clearance requests, as required.
  • Assume responsibility with consultation of the RSA, in matters of staff scheduling, and supervising, evaluating staff work performance. This will include maintaining personnel files within regulations and requirements, providing on-going and annual staff performance reviews, providing information and rationale for procedural and program concerns, counseling staff on performance/procedural issues and providing disciplinary action to staff which may include but not be limited to verbal and/or written warnings, suspension, conditional ratings, and/or dismissal.
  • Work with residential staff to assure the home operates within the goals of the program.
  • Consult and meet with the RSA on program operational and resident issues.
  • Maintain the program in concert with all policies, procedures, guidelines, and/or legal commitments to consumers as required by the Mental Health Code, Department of Human Services Licensing Guidelines, LARA, NLCMHA, and other regulatory bodies.
  • Work with the assigned Case Managers and other Interdisciplinary Team Members on program and individual concerns including development and implementation of Person-Centered Plans.
  • Monitor and assist staff in the development, implementation, documentation, and regular review of an individual’s Person-Centered Plan.
  • Assure transportation for persons living in the home to programmatic and necessary personal activities and appointments as well as to and from places of employment, day program, etc. as defined in the Person-Centered Plan.
  • Assure that medical or dental needs are identified and are met according to the Interdisciplinary Team recommendations and recognized agency procedure.
  • Assure that supplies, materials and goods are purchased in accordance with the approved budget.
  • Assure for implementation of the preventative maintenance plan; assures that the home is maintained in good repair including housekeeping and grounds care. Complete minor home repairs.
  • Assure regular staff contact with the individuals' day program staff or school, as appropriate and identified in the Person-Centered Plan.
  • Assure that all reports are submitted to the appropriate agencies, groups and individuals in a timely manner. This includes NLCMHA employee time sheets, mileage and gas logs, purchase orders, etc.
  • Foster professional development of staff through in-service education and through planning, conducting and participating in workshops.
  • Monitor, test and recommend changes in home emergency evacuation, and accident procedures/systems.
  • Maintain up to date E-Scores.
  • Maintain sufficient direct contact with the persons living in the home, their families (or other responsible party, i.e. guardian), and to be knowledgeable concerning individual needs and capabilities.
  • Represent the agency to the community, in advocacy issues, bridging relationships, and providing information.
