ACT Team Lead - Cadillac This position involves direct clinical service delivery serving the Assertive Community Treatment (ACT) Program. The position involves the development, implementation, coordination, and service delivery of a service component for NLCMHA. This position will provide direct clinical services to those served by the ACT Team, case consultation, and coordination. This individual shall strongly believe in recovery, the ability of all persons to heal, and will support these beliefs within program.

This position requires substantial knowledge of applicable guidelines, reimbursement systems, and professional knowledge of services to mentally ill and developmentally disabled consumers of the public mental health system. A demonstrated exercise of judgment is required, as well as tact in dealing with the public. This position requires the monitoring of service delivery and volume. The position requires the exercise of substantial judgment in these duties and operates under the supervision of an Adult Operations Manager or Chief Operations Officer.

Essential Functions

  • Team Lead functions are expected to approximate one-half of the full duties of the position.
  • Establishes and implements procedures for monitoring of workflow and promoting enhanced staff performance.
  • Coordinate and/or complete special projects as needed.
  • Coordinate and maintain service coverage for area of responsibility.
  • Ensure achievement of the MI Adult population Performance Indicators.
  • Ensure services delivered meet the MDCH requirements related to Evidence Based Treatment (EBT).
  • Keep informed of all significant activities, successes and problems of the area of assignment.
  • Monitor service delivery and impact of service effectiveness as well as consumer satisfaction.
  • Promote a healthy team approach to the service delivery system, manage conflicts, interpret administrative guidelines, and implement directives in a timely professional manner.
  • Demonstrates skill in managed difficult situations and prioritizing competing demands.
  • Maintain knowledge of the relationship between the programs and other services and resources within the NLCMHA system, as well as the whole human services network.
  • Act as a liaison/consultant to other programs; be a resource to staff and the community at large seeking specialized services from NLCMHA.
  • Conduct periodic needs assessments to assist in program development, maintenance of efficient effective services and accountability to consumers and groups.
  • Advocate on behalf of the specialized population receiving services.

Clinical Functions

  • ACT Team Lead direct service expectation is 50% direct consumer contact and 50% administrative.
  • Productivity requirement is 30% direct consumer contact.
  • Provide direct clinical services to registered ACT consumers.
  • Participate in the agency on-call system as a direct provider of ACT emergency services.
  • Provide indirect services for registered consumers, including case consultation and coordination, advocacy and referral.
  • Completes all necessary clinical evaluations.
  • Will consult and actively participate in decisions regarding consumers and appropriate services.
  • Participates actively in staff development activities including unit meetings, supervisory meetings, community meetings, applicable conferences and review of current literature regarding services to adults, children, and families.
  • Work cooperatively with Primary Care Physicians, local Courts, Department of Human Services, Schools, ISD, Substance Abuse and other providers to promote provision of quality community-based services.
  • Works cooperatively with other agency staff and different programs to ensure provision of quality services and transition to those services.
  • Participates in NLCMHA Compliance, Utilization and Quality, Satisfaction and Outcome monitoring.
  • Completes documentation of Crisis and Clinical Assessments, all IPOS and IPOS related documents, Progress Notes and Transfers/Discharges in compliance with documentation and contract requirements, and within time frames prescribed by NLCMHA policy.
  • Work requires irregular hours to accommodate family need and service supervision.
