Position SummaryUnder the general direction and supervision of the Medical Director and working under a contract, the Family Practice APP's primary job function is to provide clinic-based comprehensive primary care for patients of Southwest Montana Community Health Center (SWMTCHC). The APP works independently and interdependently with other clinic providers including other primary care providers, behavioral health providers, clinical pharmacy and dental providers.
Job performance is evaluated through direct observation, consultation, and written documentation. Position is Family Practice, and must be willing to see pediatric patients.
- Working within your scope of practice, provide comprehensive primary care services across the life spectrum in a primary medical care ambulatory clinic setting, including pediatrics, laboratory and diagnostic workup, specialty referrals, and urgent/emergent assessments.

- Assess patient needs and arrange for specialty referrals, diagnostic and in-patient care and make appropriate referrals for service.
- Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit.
- Verify and document that the patient understands diagnosis, treatment and follow up recommendations.
- Perform minor outpatient procedures in accordance with SWMTCHC credentialing/privileging policy.
- Work interdependently with other health care providers and health care staff in a team environment.
- Provide consultation to other clinic providers either in person or by telephone.
- Responsible for accurate, timely, and complete documentation in patient records in accordance with SWMTCHC policy.
- Provide clinical supervision and appropriate training of ancillary health care staff to assure quality of care for SWMTCHC patients.
- APPs are encouraged to participate in teaching medical students and NP/PA students.
- Participate in SWMTCHC Quality Improvement activities and any other medical staff committee activities relating to risk management or development of clinical policies and procedures.
- Participate in monthly provider meetings and routinely perform peer chart review.
- Participate in after-hours telephone call coverage when appropriate.
- Maintain credentials essential for practice, to include licensure, board certification and CME.
- Attend all required staff meetings, provider meetings, and participate in SWMTCHC committees or task force activities as directed by Medical Director.
- Advocate at local and state levels for caring for underserved/uninsured patients.
- Education: Graduate from an accredited Physician’s Assistant or Nurse Practitioner program.
- Certification: Certification as a physician assistant by the National Commission on Certification of Physician Assistants (NCCPA) or as a nurse practitioner from The American Nurses Credentialing Center (ANCC) and the American Association of Nurse Practitioners (AANP).
- License: Possess a valid license in good standing to practice as an APP in the State of Montana.
- Experience: Two years prior experience in a primary care or community health center environment is preferred; experience serving vulnerable patient populations across the lifespan is highly preferred.
- The ability to carry out functions of an APP, as described by the MT Department of Occupational Licensing and this document.
- The ability to operate a computer system including an Electronic Health Record.
- The ability to communicate effectively with patients and staff.
- Possess and apply the skills necessary to effectively treat patients of all age groups, regardless of special needs of patient.
