Main duties of the job

In this vital role, you'll be at the forefront of patient assessment, planning, and care delivery--maintaining detailed and accurate documentation both electronically and in patient‑held records, all without supervision.

Your Responsibilities:

  • Conduct comprehensive assessments and create personalised care plans, ensuring all documentation is precise and up‑to‑date
  • Manage clinic appointments effortlessly by utilising the latest IT systems within the Northern Care Alliance NHS Group
  • Coordinate follow‑up appointments for patients, streamlining their journey to recovery
  • Keep our healthcare team informed by preparing and sharing discharge letters and other essential information through our trusted IT platforms
  • Safeguard patient confidentiality with the utmost professionalism, respecting their privacy at all times
  • Strive for excellence by promoting the highest standard of patient care, making full use of available resources to benefit each individual
  • Mentor and coach junior staff and students, sharing your expertise to foster growth and confidence within the team
  • Support the Team Leader by providing education and guidance to the wider community team, including District Nursing colleague
