The purpose of the role is to provide aneffective, accurate, timely and qualitative service for processing prescription requests for registered patients.
Supportprescribing clinicians with the Practice with regards to the issuing,reauthorising and managing of prescription administration
Assistand direct patients in effectively managing and ordering medications and resolving queries and issues as required
Undertakea variety of administrative duties to assist in the smooth running of the prescribing and long-term condition management processes within the Practice, including the provision of administrative support to clinical staff and other members of the Practiceteam
Facilitate effective communication between patients, members of the primary health careteam, secondary care and other associated healthcare agencies with regards tomedication
Main duties of the job- Generate repeat prescription requests on the Practice computer system via on-line requests or repeat prescription form.
- Liaise with Practice Pharmacist/relevant prescriber regarding any requests for prescription items that are not on repeat issue or have been issued as acute previously.
- Forward all secondary care documentation to Practice Pharmacist via workflow for ongoing management.
- Identify via Practice protocols patients that are due chronic disease reviews, generating invitations via SMS message, e-mail or letter, electronic T-Quest and coding electronic notes as appropriate
- Identify patients requiring medication review and liaise with relevant prescriber regarding method of review
- Ensure correct protocols are followed regarding ordering, issuing and patient collection of all controlled drugs
- Monitor monthly prescription collection, identify any prescriptions not collected and liaise with relevant prescriber regarding further management
