Social Worker - Adult's Learning Disabilities

Responsibilities

• Have a passion for supporting adults to achieve their full potential and want to gain further experience in improving outcomes.
• Have the chance to support adults from the very start of their journey, reviewing their goals and ambitions.
• Continue to promote independence and opportunities, ensuring that these have positive outcomes.
• Undertake robust assessments, support planning, review and case management to support adults with a learning disability and/or autism ensuring that they, their families/carers and the people important to them are supported to live the life that they want to live.
• You will need a professional social work qualification (DipSW, CQSW, CSS) or other social work qualification certified by Social Work England.

• Motivated by the purpose of your role – to help and support others.

• A proven track record of a person‑centred, and asset‑focused outlook and you'll have high professional standards.
