The salary range for this position is CAD $41.42 - $59.52 / hour.

Job Summary

Royal Columbian Hospital is conveniently located adjacent to the Sapperton Sky Train station on the Expo Line, making it easily accessible by public transit for both patients and staff. The Royal Columbian Hospital Redevelopment Project (RCHRP) is a multi-year, multi-phase project to increase the hospital s capacity, beds, and services.

Responsibilities
  • Performs direct patient case management including: pre-admission screening of referred patients by interviewing patients, reviewing histories, conducting nursing assessments, and coordinating with physicians to optimize clinical conditions and ensure required diagnostic tests are booked and completed.
  • Full nursing review including diagnostic testing, identifying abnormalities, and reporting to Physicians as appropriate.
  • Provide patient/family centered education and support, using health literacy principles and translation services as needed (in person and virtual). Serve as point of contact and provide ongoing telephone support to wait-list patients and families.
  • Perform nursing assessment and patient teaching both in person and via virtual strategies.
  • Perform indirect patient case management including: communicating potential problems to clinical areas, physicians and offices; participating in daily patient reviews and triage to reflect patient demand and acuity; ensuring attendance for procedures.
  • Attend and participate in weekly Heart Team rounds as appropriate; provide information and assessment findings regarding high-risk patients to support multidisciplinary decision making.
  • Share expertise with other health care professionals; participate in triage activities and coordinate with physicians to meet waiting time guidelines.
  • Maintain tracking of patients awaiting cardiac procedures/interventions by monitoring wait times and patient condition; provide proactive patient assessment.
  • Perform administrative duties: data entry in Fraser Health and Cardiac Services BC database systems; support timely updates and reporting; contribute to quality improvement projects.
  • Other related duties as required.
Qualifications

Education, Training and Experience
Graduation from a recognized nursing program and current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM). Two (2) years of recent related clinical experience in the designated clinical area, or an equivalent combination of education, training and experience.

Clinical proficiency in the designated clinical nursing area; ability to perform initial and ongoing patient assessment; effective communication with patients, families, staff and the interprofessional care team; ability to promote person-centered care, cultural safety, and patient advocacy; data interpretation and problem-solving; teaching patients and families; organizational and prioritization skills; collaboration with the care team; use of clinical decision-making with evidence-based practice;

ability to operate related equipment and computer applications; physical ability to perform duties.
