Location: Holton Manor – Elkhorn, WI (on-site position)
Status: Full-Time | Part-Time | PRN
Flexible Scheduling - Days, PMs, Saturday

Job SummaryThis position will provide skilled therapy services for our growing Inpatient, Outpatient and Home Health Programs, and will be supported by our team of Occupational, Physical and Speech therapists. Those looking for PRN hours are encouraged to apply as well. With the ever-changing Medicare and Commercial Insurance systems, we are in need of consistent coverage to meet the needs and expectations of our customers.
At Holton Manor, our team provides therapy seven days a week and we continue to strive to grow our presence in the community. - Must uphold the professional, legal and ethical standards of practice for physical therapy as defined by the APTA and State Practice Act
- Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the department
- Performs initial patient examination, evaluation, interpretation of referrals, and creates the initial plan of care for each patient and revises as appropriate
- Creates and maintains a patient record for each patient treated
- Provides on-site assessment and re-evaluation of each patient’s treatment according to the State or Federal required interval, whichever is more stringent, and adjusts the treatment plan as appropriate
- Coordinates discharge plan and decisions with the PTA
- Demonstrates knowledge and appropriate application of precautions and contraindications to physical therapy treatment procedures and modalities
- Completes documentation according to Facility policies including timeliness and medical necessity support.
- Has primary responsibility for physical therapy care rendered by the PTA for all patients/clients for whom he or she is the therapist of record. In doing so:
- Delegates appropriate portions of the treatment plan and program to the PTA consistent with the PTA’s education, training and experience
- Must be always accessible by telecommunication to the PTA while the PTA is treating patients/clients for whom he or she is the therapist of record
- Performs other duties as required and/or assigned
- Graduate of an accredited education course in physical therapy.
- Licensed in the state of Wisconsin.
- Must not be excluded or sanctioned from state or federal program.
- Demonstrate professional attitude and appearance.
- Proficient administrative and communication within department and facility and company.
- Thorough knowledge of the principles, methods, materials, and equipment used in the practice of their discipline.
- Display working knowledge of multidisciplinary practice scopes in the Outpatient, ALF, ILF or LTC setting.
- Thorough knowledge of all payer source documentation and payment requirements.
- Ability to use analytical skills to develop outcome driven solutions.
- Prior EMR/computer experience
- Must meet all health, background check, and training requirements as required by Wisconsin law
- Hiring manager contact within 1–2 business days
