Substance Use Case Manager – Statewide

This unique and dynamic Case Manager role is perfect for someone who enjoys variety in their day-to-day work, is energized by new environments, and is passionate about making an immediate, tangible impact. This Case Manager role is designed to temporarily cover caseloads in various Vermont regions, ensuring continuity of care when staffing gaps arise and providing additional support during times of high-volume needs—while building strong partnerships with DCF offices, treatment providers, and community supports.

Our Regional Partnership Program (RPP) works in close collaboration with the Vermont Department for Children and Families (DCF) to support families impacted by parental substance use. This role provides additional support to RPP Case Managers Statewide. Responsibilities may include but not limited to:

• Provide short-term case management coverage across different districts, stepping in during staff transitions, leaves of absence, or when needs increase regionally.
• Conduct compassionate and confidential screenings for substance use concerns.
• Help parents take meaningful steps toward recovery by linking them with assessments, treatment services, and community-based supports.
• Work side-by-side with DCF Family Services staff to support safety planning, family reunification, and long-term stability.
• Address common barriers to engagement, such as transportation, childcare, and stigma—meeting families where they are, both literally and emotionally.
• Share your knowledge with others by consulting with DCF teams and offering informal trainings on treatment systems and resources.
• Serve families in a variety of settings: DCF offices, homes, and community spaces—wherever the support is most needed.

This is a full-time position and is based out of Lund’s South Burlington office. Position may include some travel to various districts throughout the state of Vermont. The starting rate is $25/hr for the full job description, .

• Minimum of an associate degree required. A bachelor's degree in social work, counseling, or a human services field is preferred.
• Experience providing case management to persons in a substance use, recovery, or mental health treatment setting preferred.
• Apprentice Addiction Professional (AAP) certificate or the ability to test for certification within first year of employment. Financial support, clinical supervision, and training provided by Lund to obtain the AAP certificate.
• Experience working with families, in multidisciplinary teams, and knowledge of community resources preferred.
• Ability to adhere to strong professional boundaries and maintain client confidentiality in line with state and federal regulations.
• Skilled in the use of technology, including Microsoft Suite programs, Electronic Health Records and video conferencing (Zoom and Teams)
• Regular attendance is necessary and essential to meeting the expectations of the job.
• Ability to maintain a valid VT Driver’s License and access to reliable transportation is required; occasional statewide travel may be required.
