Licensed Practical Nurse (LPN) - Full Time and Part Time Opportunities Job Summary

Birchwood Terrace Rehabilitation and Healthcare is seeking highly motivated and compassionate Licensed Practical Nurses (LPNs) to join our dedicated team of caregivers. • Supervise Certified Nursing Assistant (CNA) staff on assigned units
• Deliver care in accordance with Facility protocol and safe nursing practices
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Provide direct patient care, including administering medications, wound care, and other treatments as needed
• Maintain accurate and detailed records of patient care and progress

• Current unencumbered State of Vermont LPN license
• Strong communication skills to communicate with residents, families, and staff
• Critical thinking skills to problem-solve and make informed decisions
• Ability to handle different situations with tact and professionalism
• High school diploma or equivalent required
