The Bleeding Disorder Clinical Program Coordinator is responsible for clinical and programmatic coordination within Hematology Services. This role manages the multidisciplinary care of patients with bleeding disorders such as Hemophilia A and B, von Willebrand disease, and other rare coagulation disorders. As a clinical leader and nursing expert, the coordinator provides and coordinates care and education to patients, families, and the community.
Key Responsibilities- Provide and coordinate clinical care and patient follow‑up.
- Deliver patient and family education on disease management.
- Manage case coordination and support.
- Develop and improve program initiatives.
- Engage in community outreach and advocacy efforts.
State of Vermont Registered Nurse (RN) license required.
ExperienceBleeding disorder RN experience preferred.
