The Converse Home
, a warm and welcoming assisted living community in Burlington, VT, is seeking part-time and per diem Licensed Practical Nurses (LPNs) to join our team.
In this role, you will use your clinical skills and passion for caregiving to make a meaningful difference in the lives of our residents each day. We are currently hiring for the evening shift (2:30 PM–11:00 PM) on our Memory Care community
, as well as per diem LPNs with flexible, as-needed availability.

Key Responsibilities- Provide high-quality nursing care with compassion and professionalism.
- Administer medications and treatments in accordance with care plans and physician orders.
- Monitor residents’ health status and promptly report changes to the healthcare team.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Educate and support residents and their families.
- Maintain accurate, timely, and thorough medical documentation.
- Current LPN license in the state of Vermont.
- Experience in assisted living, long-term care, or a related setting preferred.
- Strong organizational and time-management skills.
- Excellent communication and interpersonal skills.
- A team-oriented, patient-centered, and compassionate approach to care.
