Overview
The Case Manager (CM) coordinates the care and services of selected patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; and collaborates with an interdisciplinary team to achieve patient-centered clinical and resource outcomes. They proactively identify and intervene to address barriers to treatment, health outcomes, and discharge planning, including biopsychosocial and other high-risk factors.
They are an essential link between the patient, the healthcare team, the payer, and the community.
- Coordinate care and services for selected patient populations across the continuum of illness
- Promote appropriate utilization and monitoring of health care resources
- Intervene to address barriers to treatment, health outcomes, and discharge planning
- Collaborate with an interdisciplinary team to achieve optimal patient-centered, clinical, and resource outcomes
- Identify, assess, and address biopsychosocial and other high-risk factors
- Serve as a key liaison among patients, healthcare teams, payers, and community resources
- Master s in social work from an accredited school of social work. LICSW preferred.
- Or current RN licensure or compact licensure recognized by the State of Vermont
- Or Master s degree in a Health or Human Services field (e.g., Counseling) with licensure preferred
- Or Master s degree in an Accredited Occupational Therapy Program; licensed or eligible to practice OT in Vermont
- Or Bachelor s of Science or higher degree from an Accredited Physical Therapy program; licensed or eligible to practice PT in Vermont
- Case management accreditation (CCM, ACM, ANCC) preferred
Minimum of 3 years of experience in a clinical setting, including 2 years in a medical setting (preferably a hospital). Two years of case management experience or a similar role preferred.
