- Doctorate in Physical Therapy (DPT) from an accredited program
- Current and active Physical Therapist license in North Carolina
- Minimum 1 year of experience working with frail or elderly populations preferred
- Strong clinical competency in geriatrics, rehabilitation, and mobility improvement
- Excellent communication, assessment, and documentation skills
- Must meet CMS-approved competency standards prior to independent practice
- Proof of medical clearance for communicable diseases and current immunizations
Rewarding opportunity for a Physical Therapist (PT) to join a Program of All-Inclusive Care for the Elderly (PACE) team in Burlington, North Carolina. This full-time role supports older adults through comprehensive, interdisciplinary physical therapy services designed to enhance mobility, independence, and quality of life.
Job OverviewThe Physical Therapist provides evaluation, treatment, and ongoing care planning for adult and geriatric participants enrolled in a PACE program. This position focuses on functional improvement, fall prevention, and restorative therapy in both day center and home settings. Working as part of an interdisciplinary care team, the PT ensures every participant receives individualized, coordinated, and compassionate care.
Key Responsibilities- Conduct comprehensive physical therapy evaluations and scheduled reassessments
- Develop and implement individualized therapy plans targeting mobility, strength, and functional goals
- Plan and lead maintenance and group exercise programs for ongoing therapy
- Evaluate neuromotor and musculoskeletal function and recommend adaptive or assistive devices
- Participate in interdisciplinary team (IDT) meetings and contribute to care planning
- Document progress notes, assessments, and discharge summaries per policy
- Educate participants and caregivers on safe movement techniques and home exercise programs
- Support infection control, safety initiatives, and quality improvement activities
