About Position:

The Enrollment Specialist is responsible for the enrollment of new participants to the program, including all intake activities. Under the supervision of the Marketing and Enrollment Manager, the specialist obtains initial information relating to admissions, coordinates the intake and enrollment process, and provides information and referrals to community agencies as appropriate. The specialist promotes the PACE program to potential participants, community agencies, and at other outreach venues as needed.

Schedule:

Monday through Friday, 8:00am to 5:00pm

Duties/Responsibilities –
  • Evaluates potential participants to determine needs and eligibility for enrollment.
  • Conducts assessments in the homes of potential participants according to availability, including evenings and weekends as needed.
  • Utilizes knowledge and required skills to ensure appropriate assessments.
  • Assesses individual patient conditions using clinical observation, medical records, verbal information, interactions with patients/families and the care team.
  • Coordinates level‑of‑care applications with State Medicaid (or designee) and Medicare applications with county Department of Social Services personnel.
  • Participates in IDT, Management Team, and related meetings and activities.
  • Participates in outreach activities in conjunction with the Marketing and Enrollment Manager.
  • Completes documentation related to participant assessments as required.
  • Maintains required statistical data.
  • Presents intake reports at the weekly staff meeting and as needed.
  • Travels to client homes and/or other agencies in the community as needed.
  • Possesses knowledge of NC’s nursing facility level‑of‑care criteria (FL2).
  • Collaborates with the Nurse Care Manager and Team Director to oversee execution/signing of Care Plan and Enrollment Agreement.
  • Participates in outreach events and presentations in the community.
  • Provides alternative community resources to applicants not accepted into the PACE program.
  • Contacts identified referral sources regularly to discuss possible program candidates.
  • Maintains ongoing relationships with identified individuals and agencies to ensure a clear understanding of PACE as a vital community resource.
Qualifications:

Education/

Experience:

3‑year degree from a recognized college or university, or an equivalent combination of experience. Strong preference for knowledge of the community served by Piedmont. Preference for sales experience and demonstrated success in meeting sales benchmarks. One year of experience with the frail or elderly preferred.
Bilingual/Spanish speaking candidates are strongly preferred.

Licensure, Registry or Certification

Required:

Valid NC Driver License and CPR/BLS certification.
