SUMMARY OF POSITION

The Care Coordinator Medical Interpreter is part of the NEMS MSO Care Coordination and Case Management (CC/CM) team. The primary function is to establish communication between health care providers and NEMS patients who have limited English proficiency or prefer to speak in their native tongue. The interpreter will interpret and translate complex medical information to patients and ensure complete understanding across medical professionals and patients.

This position requires virtual system usage as well as face‑to‑face service at various medical facilities, such as physician offices, hospital outpatient facilities, nursing homes, and rehabilitation centers.

DESCRIPTION

This role is an essential communication link between doctors and patients. Candidates must have effective communication skills to avoid cultural misunderstandings, foster a positive and relaxed experience, and uphold patient confidentiality per NEMS policies. They should understand managed care benefits and coverage (Medicare Advantage, D‑SNP, Medi‑Cal Managed Care, and other Supplemental programs) to facilitate timely, efficient healthcare services. The interpreter will also coordinate medical, behavioral, social, and community support services, developing multidisciplinary care plans, self‑management goals, and referrals to internal and external resources.

ESSENTIAL

JOB FUNCTIONS
  • Accompany non‑English speaking patients to specialist appointments or provide virtual services; interpret and translate medical information—conditions, treatments, follow‑up details, insurance coverage—during visits.
  • Translate medical documents such as medical history questionnaires, post‑appointment or discharge summaries, self‑care instructions, and health education materials before, during, and after appointments.
  • Provide accurate interpretation between patients and healthcare providers; ensure mutual understanding.
  • Access virtual systems to deliver interpretation services.
  • Coordinate post‑appointment care to ensure referrals to other specialists or procedures are arranged.
  • Monitor care delivery and progress toward desired outcomes; negotiate scheduling of tests, procedures, and consults with service providers and facilities.
  • Participate in interdisciplinary care team meetings to evaluate care outcomes, measure intervention effectiveness, and implement team recommendations.
  • Serve as the system navigator and contact point for patients enrolled in managed‑care programs, assisting with benefit coverage and access questions.
  • Provide enhanced care coordination, helping patients solve issues related to the healthcare system, finances, or social barriers.
  • Conduct outreach activities to assigned members by phone, mail, or other communication methods to promote program engagement and NEMS marketing strategy.
  • Collect and report statistical information for monthly and quarterly reports.
  • Document program measure results and submit to the MSO Committee as needed.
  • Contribute to NEMS marketing strategy and outreach project planning.
  • Perform other job duties as required by manager or supervisor.
QUALIFICATIONS
  • Completion of a two‑year degree from an accredited higher education institution.
  • Certification from an accredited Medical Interpreter program or Healthcare Interpreter Program in Cantonese and/or Mandarin, Vietnamese, or Spanish; may be substituted by passing the Language Line Interpreter Skills Test (IST).
  • 1–3 years’ experience interpreting in a clinical setting preferred.
  • Strong knowledge of medical terminology.
  • Excellent listening, speaking, and retention skills to convey messages accurately.
  • Excellent social and communication skills, strong analytical and problem‑solving skills.
  • Valid driver’s license and ability to operate a company vehicle.
  • Knowledge of State/Federal healthcare compliance requirements, particularly DHCS/Medi‑Cal guidelines, a plus.
  • Ability to self‑manage in a detail‑oriented environment.
  • Ability to operate PC‑based software programs or automated database management systems.
