Registered Nurse Case Manager
Location:
Burbank, CA
Duration: 13 weeks
Shift: Night Day 5x8-Hour (08:30 - 16:30)
Responsibilities of Registered Nurse Case Manager
- Conduct diagnostic evaluations and assessments to determine patient needs and develop individualized care plans.
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with procedures.
- Collaborate with physicians and other healthcare professionals to coordinate patient care and ensure continuity of services.
- Educate patients and their families on disease management, treatment options, and self-care techniques.
- Monitor patient progress and make adjustments to care plans as necessary.
- RN License
- 1 year experience in Case Management
