Unit Description – Case Management Department
Unit Type:
• Case Management (Inpatient focus)
Workload & Patient Flow:
• Caseload up to 25 patients per day
• Supports adults and geriatric populations across multiple service lines
Shift Details:
• 8-hour day shifts: 08:00 AM – 04:30 PM
• 4-week schedule designed to meet departmental needs
• Weekend requirement: typically one full weekend per 4-week period (may vary by needs)
• Call and floating required based on staffing
Primary Responsibilities:
• Perform comprehensive assessments and determine care needs
• Lead care coordination and manage all aspects of discharge planning
• Collaborate with IDT to remove barriers to care, expedite progress, and ensure safe transitions
• Conduct concurrent/continued-stay reviews and ensure documentation aligns with payer expectations
• Work with HMOs, IPAs, and insurance partners to manage authorizations and avoid denials
• Use standardized criteria such as Inter Qual and Milliman
• Order DME, arrange rehab/SNF placement, home health, and transportation services
• Maintain accurate, timely documentation within Epic or comparable EMR
• Communicate using standardized SBAR format
Required Experience & Expertise:
• Minimum 2 years acute hospital Case Manager experience (care coordination + discharge planning)
• Strong proficiency with:
- Level-of-care criteria (Inter Qual/Milliman)
- HMO/IPA workflows
- Discharge planning for medical-surgical populations
- Regulatory knowledge: CMS, HIPAA, Joint Commission, DRG principles
• EMR proficiency (Epic preferred)
