Position Summary
The Care Liaison Manager - PAH is responsible for modeling Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and promoting the Compassus philosophy using the 6 Pillars of Success. Position Specific Responsibilities- Lead implementation of care coordination activities for each Home and Community Care (HCC) service line, including Hospice and Home Health, within the geographic area of responsibility.
- Represent all continuum ministries in the geographic territory and maintain core relationships within the hospital system to ensure collaboration around shared VBE goals.
- Facilitate operational improvements through participation in management and improvement meetings for all ministries.
- Coordinate services across settings including acute care, physician/ambulatory, and Providence Health Plan in furtherance of the value‑based enterprise goals.
- Provide leadership, oversight, and training for clinical liaison and CTA staff and foster strong collaboration among all service delivery components.
- Develop, implement, and lead strategic and tactical care coordination plans and service enhancement growth plans for each service line.
- Ensure collaboration and alignment around established shared goals.
- Required:
Bachelor’s Degree in a healthcare‑related field, Business Administration, Communications, or equivalent education/experience. - Preferred:
Master’s Degree in Clinical, Business, Health Administration, or related area. - Required:
3 years of complex healthcare business project management experience. - Preferred: 3 years of documented prior success in managing healthcare teams, business development, or program management.
- Preferred:
Experience collaborating directly with healthcare executives and decision makers.
- Mathematical skills: ability to perform calculations with whole numbers, fractions, decimals, and compute rates, ratios, and percentages.
- Language skills: ability to read, analyze, and interpret business journals, technical procedures, and regulations; write reports and business correspondence; and present information effectively.
- Other skills: strong interpersonal and customer‑service skills; ability to work independently and collaboratively; proactive problem‑solving; high organization and detail orientation; travel required.

The role requires regular standing, walking, and manual dexterity, with the ability to lift objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and focus adjustment. Exposure to bodily fluids, infectious diseases, and standard medical equipment is expected.

CompensationRange
$142,200 – $173,800 annually.
