Ascend Medical is seeking a compassionate and experienced Nurse Practitioner or Physician Assistant to provide relationship-centered care at premier Senior Care communities in Sugar Hill and Grayson, GA. This part-time role is ideal for a provider who is passionate about geriatrics and values quality time with patients over quantity.

Schedule
  • 1.5 days (12 hours) per week
    Full day 8a-4p
    Half Day 8a-12p
  • Additional day (8 hours) in Grayson location beginning April.

  • Build trust with patients, families, and facility staff through clear communication and consistent presence.
  • Manage chronic conditions, address acute concerns, and tailor care plans to the unique goals of each patient.
  • Collaborate with the facility’s care team to ensure seamless, coordinated support.
  • Apply your home health or hospice experience to improve outcomes for medically complex patients.
  • Maintain flexible availability for patient visits and on‑site presence.

Requirements
  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Georgia (unrestricted, non‑probationary).
  • Master’s degree from an accredited program.
  • Minimum of 3 years of clinical experience, with strong preference for geriatric, hospice, or home health background.
  • Active DEA license (or eligibility to obtain).
  • Availability to work on‑site in Sugar Hill and Grayson (April); occasional visits to additional locations may be required.
