The Intensive Family Coordinator will provide a professional level of case management services as well as direct advocacy services to a caseload of families.

• Complete family assessments and create individualized service plans.
• Provide services for the truancy program in the school district.
• Report to the Director of the Program.
• Utilize informal supports drawn from resources in client families’ home neighborhoods to build a network of support for each youth and family referred.
• Travel as necessary; willingness to work nights and weekends; provide 24/7 crisis support to the caseload.

• Minimum High School Diploma or GED Equivalent.
• Experience working with youth.
• Excellent verbal and written communication skills.
• Proficient in Microsoft Office 365; experience using an Electronic Health Record (EHR) system is a plus.
• Bilingual/Spanish speaking is a plus.
• Reliable transportation, valid driver’s license, and current auto insurance coverage is required.
