This is a part‑time position in the Admissions Department. 8 a.m.–4 p.m. every Saturday and Sunday. Holiday rotation.

A Licensed Practical Nurse (LPN) Coordinator handles day‑to‑day coordination of the team’s clinical functions, assists in implementing the patient plan of care with staff, volunteers, and outside agencies. Communicates as a liaison between staff, patients, and outside agencies. Assists with maintaining records. May involve direct patient care.

• Assists in coordination and implementation of the patient plan of care.
• Handles phone calls from the patient, primary caregiver, or designee regarding any questions or concerns.
• Collaborates with interdisciplinary teams, RNs, or Team Manager.
• Coordinates and schedules patient visits.
• Provides timely documentation/data entry per Hospice time frames and deadlines.
• Communicates current health status and needs of patients to appropriate team professionals.
• Assists the team and Team Manager with other administrative functions, as necessary.

Education/Certification: Graduate of an accredited School of Practical Nursing with current LPN licensure in the State of New York.

Experience: 1 year of nursing practice as a Licensed Practical Nurse preferred.

Salary: $27.00 - $33.00 per hour.
