Overview

Seeking a Nurse Practitioner to deliver equitable, value-based primary care to Medicare adults in outpatient settings.

• Conduct office visits, wellness exams, care coordination, health education, and participate in population health initiatives.
• Collaborate with a multidisciplinary team and may include home visits and outreach activities.

• Master’s or DNP degree and national certification in family, adult, or gerontological primary care
• Active NP license, DEA registration, and US work authorization
• Preferred experience in primary care, HCC/ICD-10 documentation, and bilingual skills
