Coordinates the total Plan of Care for the patient in the Home Care setting, assessing, planning, implementing and evaluating. Provides leadership for the Health Care team by working cooperatively with other team members to maintain standards of professional nursing practice in the Home Care setting.

Coordinate care plans, assess patient needs, implement nursing interventions, evaluate outcomes, and lead the health care team to uphold professional nursing standards within the home care environment.

Associate Degree in Nursing required. Graduate of an accredited School of Registered Nursing. Current licensure to practice as a Registered Nurse in NYS. Valid NYS driver’s license. 1-5 years of relevant experience, including at least one year of medical/surgical RN experience and one year of infusion experience.

$40.80 - $50.90. Wage will be determined based on factors such as candidate experience, qualifications, internal equity, and any applicable collective bargaining agreement.
