Nurse Practitioner - Spanish

Overview

The Nurse Practitioner role involves providing equitable, value-based primary care to adult Medicare patients in outpatient settings.

• Independently conduct routine and acute visits, wellness exams, and health assessments.
• Collaborate with a multidisciplinary team including physicians, medical staff, and specialists.
• Lead care coordination, patient education, and population health initiatives.
• Participate in outreach, phone triage, and home visits as needed.
• Report to the Center Medical Director and work closely with operational leadership.

Qualifications include a Master’s or DNP degree, national certification, active licenses, and DEA authorization. Preferred experience in primary care, geriatrics, or family medicine, with knowledge of coding and risk assessments.
