Overview

We are seeking a Nurse Practitioner to deliver equitable, value-based primary care to Medicare adults in outpatient settings.

• Provide comprehensive primary care, conduct routine and acute visits, and perform wellness assessments.
• Collaborate with a multidisciplinary team including physicians, nurses, and support staff.
• Coordinate care with specialists and community resources, educate patients, and participate in population health initiatives.
• Engage in outreach, phone triage, home visits, and promotional activities.

Qualifications include a Master’s or Doctorate in Nursing, national certification in family, adult, gerontological, or primary care, and active licenses. Preferred experience in primary care and understanding of health documentation is advantageous.
