. This is a part‑time position in the Admissions Department, 8 am‑4 pm every Saturday and Sunday, with holiday rotation.

A Licensed Practical Nurse (LPN) Coordinator handles the day‑to‑day coordination of the team clinical functions, assists in implementing the patient plan of care with staff, volunteers, and outside agencies, communicates as a liaison between staff, patients, and outside agencies, assists with maintaining records, and may involve direct patient care.

Essential Duties and Responsibilities
  • Assist in coordination and implementation of the patient plan of care.
  • Handle phone calls from the patient, primary caregiver, or designee regarding any questions or concerns.
  • Collaborate with the interdisciplinary teams, RNs, or Team Manager.
  • Coordinate and schedule patient visits.
  • Provide timely documentation/data entry per Hospice time frames and deadlines.
  • Communicate current health status and needs of patients to appropriate team professionals.
  • Assist the team and Team Manager with other administrative functions, as necessary.
Qualifications
  • Graduate of an accredited School of Practical Nursing with current LPN licensure in the State of New York.
  • 1 year of nursing practice as a Licensed Practical Nurse preferred.

Salary: $27.00 per hour
