We are seeking a compassionate Nurse Practitioner (NP) for a Skilled Nursing Facility in Johnstown, NY. This full‑time role offers flexible scheduling, including part‑time and per diem options, with a typical Monday–Friday schedule and some weekend/holiday coverage.
Key Responsibilities- Conduct assessments, develop treatment plans, manage medical conditions, and prescribe medications.
- Collaborate with interdisciplinary teams to ensure high‑quality resident care.
- Document in the EHR, participate in care team meetings, and support nursing staff.
- Active NY NP license.
- Certification as an Adult‑Gerontology or Family Nurse Practitioner.
- DEA registration.
- BLS certification.
- Preferred experience in geriatrics or long‑term care.
