Coordinates the total Plan of Care for patients in the Home Care setting, assessing, planning, implementing, and evaluating. Provides leadership for the Health Care team by working cooperatively with team members to maintain standards of professional nursing practice.

• Provide leadership for the Health Care team, working cooperatively with other team members.

• Associate’s Degree in Nursing.
• Current RN license required.
• 1–5 years of relevant experience.
