FT & PT positions with Primary Care group practice with 2 locations. Flexible scheduling, no nights, weekends, or holidays! Competitive salary with PTO benefits, 401K and profit sharing. Current New York State Professional Nurse Registration is required. Graduate of an accredited school of professional nursing with an RN/BSN degree in nursing. At least 1 year of nursing experience is required. Medent EMR experience is necessary.
As an advocate for the patient, the nurse care coordinator will assess, implement, coordinate, monitor, and evaluate the options and services required to meet an individual's health needs, using clinical and community resources to promote quality and cost-effective outcomes. Provides targeted interventions to avoid hospitalization and emergency room visits.
