Working hours:
8:00 a.m. – 4:00 p.m., Monday – Friday with holiday rotation.

- Provide skilled nursing care to patients as directed by the IDG plan of care in accordance with agency policies, including personal care, vital signs, and medication administration.
- Document patient care to agency standards with timely completion and signatures, following the A-I-R protocol.
- Reinforce patient and family teaching regarding disease process, self-care techniques, and prevention strategies under the guidance of the assigned registered nurse.
- Assist in planning and exchanging information with other health professionals to maintain continuity of patient care.
- Assist the physician and registered nurse in performing specialized procedures.
- Identify and report patient or family complaints or unusual occurrences, including incident reporting.
- Demonstrate proper sharps disposal, hand hygiene, waste disposal, and use of personal protective equipment.
- Communicate current health status and patient needs to appropriate health professionals, contributing to the coordination of the overall care plan and discharge planning.
- Participate in quality improvement programs and document required quality assurance data.
- Participate in weekend, holiday, and backup visits as needed.
- Adhere to agency policies and procedures focused on patient care and organizational administration.
- Maintain compliance with HIPAA regulations and ensure security of patient electronic and written records.
- Graduate from an accredited School of Practical Nursing with current LPN licensure in the state of New York.
- Valid driver's license with no serious violations; vehicle required for on-duty or on-call use.
- Preferred: 1 year of nursing practice as a Licensed Practical Nurse.
Salary: $27.00 per hour.
