This role delivers comprehensive home‑based primary care across Erie County, NY, while collaborating with both Emcara Health and Lucet teams to support patient and member journeys.

• Annual compensation $125,000–$135,000 plus discretionary performance incentive.

• Conduct a minimum of 25 in‑home visits per week, including assessments, exams, and medication reviews.
• Manage chronic conditions and develop individualized treatment plans.
• Serve as the primary care provider and conduct follow‑ups.
• Maintain accurate EMR documentation and comply with healthcare regulations.
• Educate patients on disease management and healthy living.
• Collaborate with interdisciplinary teams to ensure coordinated care.

• Active, unrestricted licensure in the state of practice, clear of clinical and/or board practice restrictions.
• Certification as a Family Nurse Practitioner, Adult‑Gerontology Nurse Practitioner, or Physician Assistant.
• 1+ year of experience in primary care or related outpatient settings.
• DEA license and BLS certification.
• Strong clinical and diagnostic skills.
• Proficiency in electronic medical record (EMR) systems.
• Reliable transportation and clean driving record.
• Ability to pass comprehensive background checks as outlined in the description.

The position requires meeting patients in their homes throughout Erie County, NY, with a territory radius of 50 miles from your home address.
