Job Type: Full-time (Evening Shift 3p-11p)

Pay Range $28.00-$32.00/hour Plus Sign-On-Bonus up to $3,000

Lead with confidence. Grow with purpose. Garden Gate Skilled Nursing & Rehab is seeking a reliable and experienced Licensed Practical Nurse (LPN) Unit Coordinator to lead a dedicated nursing unit.

• Coordinate daily clinical operations for the assigned unit
• Provide hands-on nursing care and oversee medication and treatment administration
• Supervise and support CNAs and unit staff to ensure high-quality resident care
• Monitor documentation, care plans, and compliance with regulatory standards
• Serve as a point of contact for families, physicians, and interdisciplinary team members
• Participate in staff scheduling, training, and performance discussions
• Support communication between nursing leadership and frontline staff

• Current LPN license
• Minimum of 1 year experience in long-term care or skilled nursing (Preferred)
• Strong leadership, organizational, and communication skills
• Ability to effectively delegate, problem-solve, and prioritize care
• Experience in a lead nurse, charge nurse, or team leader role preferred
