Overview
Seeking a Speech Language Pathologist for our Home Care team to provide individualized evaluations and treatments in patients' homes, focusing on communication, cognition, and swallowing to promote safety and independence.
Responsibilities- Interpreting prescriptions, conducting assessments, developing and adjusting evidence-based treatment plans, documenting progress, and educating patients and caregivers.
- Collaboration with interdisciplinary teams and participation in care planning.
- Master’s degree in Speech-Language Pathology, CCC or CFY, NY State license, CPR certification
- Preferred experience with dysphagia and neurogenic disorders
